• Home Health Care
Companies that provide skilled intermittent care to patients in a home setting rather than in a medical facility. Home health care may include skilled nursing such as wound care or diabetes management, physical therapy, occupational therapy, psych nursing, and/or speech therapy. Home Health Care may also provide a Home Health Aide for a period of time to assist with personal care needs a few days a week for about an hour at a time.
• Skilled Nursing Facilities
Facilities qualified to provide skilled nursing services under the supervision of licensed nurses. Skilled nursing care includes rehabilitation and various medical and nursing procedures. These facilities may also be known as a long term care facility or nursing home.
• Alzheimer’s/ Dementia
Programs and or facilities offering specialized programs for clients/residents living with Alzheimer’s/dementia, or any other form of memory loss. These programs can be offered by Residential, Assisted Living, or Nursing Facilities.
• Hospice Services
Hospice Care may be provided in the home or within an inpatient hospice facility. These services include Palliative Care or pain management and provide the client and families with professionals that may also attend to the emotional and spiritual needs of terminally ill patients. |
